2/13/2018 - Families of Deceased Past Employees

To the families of deceased past employees,

Families or Estates of deceased past employees of the Lakeview School District, who are eligible for the 3% refund from July 1, 2010 - September 3, 2012, ORS healthcare withholding should have received notification from ORS.  Please provide to the district either court papers or a written statement of your relationship to the deceased and any surviving family members.  Please sign your written statement and send it to the address given below.
