• Welcome to Lakeview School District!

    Please complete the following steps to enroll your student.  

    Step 1 - Please complete the online NEW STUDENT ENROLLMENT APPLICATION.

    Step 2 - The following documents are required to complete the enrollment process.  Please collect this information in preparation to upload it into the online enrollment system.

    • Child's Birth Certificate
    • Proof of Residency - One of the following for your child's home address: gas, water, or electric bill; rental agreement; mortgage statement; tax bill (bills must include the date and be current within the past 30 days)
    • Parent or Guardian Photo ID (Name on Photo ID should match name on Proof of Residency)
    • Immunization Records
    • Vision and Hearing Screening Results (Kindergarten only)
