Over the years, Lakeview has enjoyed recognition for providing an excellent education and values to its students. Many Lakeview High School graduates have excelled in their professional and personal careers, as well as being involved in their community.
The Distinguished Alumni Award recognizes a Lakeview High School graduate who has graduated at least 20 years earlier and who has distinguished him/herself, bringing positive recognition to Lakeview Schools. The Board seeks a candidate who has distinguished him or herself through community service and personal and professional accomplishments.
Nomination applications will be available beginning January 21, 2010 and will be due by 3pm on Friday, March 5, 2010. Nomination packages are kept for two years from the year of submission. All nomination packages submitted to the Lakeview Superintendent of Schools will be reviewed by the Distinguished Alumni Committee, who will then provide a recommendation to the Board of Education. Final selection of the Distinguished Alumnus will be made by the end of March with the awardee notified at that time.

1. Type into the pdf form* and, when you are done, click the "Submit Form" button at the top of the form. You will receive an email within 24 hours (Monday-Friday) confirming receipt of your application. If you do not, please call (269) 565-2411 to ensure that your application was received.
2. Type into the pdf form* and, when you are done, print the form and mail it to:

The deadline for submission is 3:00 p.m. on Friday, March 5, 2010.
*The pdf form can be saved to your hard drive to work on at your leisure or so you can save a personal copy. If necessary, you may print a blank form and hand-write your responses then mail the form (see option 2).
